“BABY SITTER CONSENT FORM”

(CONSENT OF PARENT OR GUARDIAN FOR TEMPORARY CUSTODY AND EMERGENCY MEDICAL TREATMENT OF MINOR CHILD)Baby Sitters Permission

The purpose of this document is to provide permission for the care and custody of a minor child. You may use it any time you have custody of someone else’s child or leave your child in the care of another person, like a baby sitter. When properly used, this form authorizes the named person(s) to:

  • Act as the child’s temporary custodian. This means the custodian can sign forms that require a parent’s signature such as registering for school, sports, clubs, or field trips.
  • Perform or authorize first aid and other medical care for the child in their custody. This may save a child unnecessary suffering by removing delays.
  • Administer appropriate discipline. The parent or guardian authorizes the type of discipline. This way, the discipline given is what the parent wants.

It is important to complete the form properly. The more complete and accurate the form, the more effective it is, and the lower the risk of a problem. The Termination Date may be for any duration of time up to the child’s eighteen birthday.

The document does not need to be notarized to be effective.
However, if it is notarized it authenticates the document to make it more acceptable to hospitals, doctors and schools.

It is important to complete a form for each child and for each person who may have custody of each child. It is generally best to complete the form in advance of an emergency for multiple appropriate people which may include responsible family members and friends. You may use this form any time you have children in your care, act as chaperone, or host of a child’s activity when the parents do not attend. You may make copies as needed and share this form with others.
